How would one pass from car to car through a conventional full width cab? Moreover, what's the need to do that? I assume that it might be an issue in an emergency, but this design would probably implement some kind of fold-out emergency end door to include that.

Also, virtually all vehicles with centrally-mounted operating positions have one large window in the front for the operator to operate from. Not only does this increase visibility, it doesn't make the wayside signals any less visible.
