Tuscarora Almanac for July 25

1990 from The Book of Wrecks

A northbound “M” train keyed by signal on the West End Line near Fort Hamilton Parkway. It collided with the “B” train in front of it. The “M” train had cars (n) 4821-0, 4819-8, 4817-6 and 4815-4.The consist of the “B” train was (n) 4283-2, 4169-8, 4346-7, 4334-5 and 4407-6. It was reported that the force of the collision was so great that the crews could not at first uncouple the trains. They were finally able to do so by removing the drawbar between 4346 and 4347.

How does something like this happen? Is it completely t/o error? Doesn't the t/o need permission to key by? And isn't keying by done at a slow speed? And then the train would have to speed up to cause such a serious collision? All on open outside track with clear views ahead?

Was that t/o injured? Fired?

That's the scariest part of "keying by/bypass" red signals.

If you are Seasoned, attentive, know Your Iron,then you will be able to do this with out much problems.

But some don't pay attention as well, so the practice has become a HUGE No Mo now.

You STOP AND STAY, UNTIL THE SIGNAL CLEARS.

Even though theT/O had permission, there is a visibility problem at that location. As a result, the signal was rewired so that the key by feature was completely disabled.

Looks like it happened a second time near the same location on February 9, 1995. Here's the NTSB report on that one.
