"Prominence" = celebrities, elected officials, major religious leaders

Sexual orientation: there is still plenty of anti-LGBTQ bias in the US healthcare system. For example, gay men are stilbilitil prohibited from donating blood if they've had sex with another man in the past year. This is still in effect, despite today's extensive blood screening and testing capabilities. LGBTQ patients in general often encounter judgmental doctors and nurses, and the value judgment of these providers can and does impact their care. (How do doctors and hospitals find out if you're LGBTQ? If you're gay or lesbian, you might list a same-sex partner as your emergency contact. If you're trans, it's part of your medical history. )

Language barriers are way down the list, because they are common, especially in big cities like NYC. Before that becomes a decisive factor in your care, the other five criteria I mentioned, will have already decided your fate.
