And as others have said, seeing NO signals in front of her, this T/O should not have moved without clear instructions first. |

The old signals are gone and AFAIK, the BMT is 100% "new" signaling. I am including the Bway Subway which while being controlled from City Hall Master Tower made use of some of the old BMT signals that were wired into the new system. |

The Willy B tracks are not reverse signalled, but the are protected by interlockings at Marcy Av and Essex St. Consequently, when doing single track operation, they can have have only one train at a time on the bridge, necessitating 24 minute headways. |

The train operator misunderstood the instructions that she was given. She was reportedly told to pull her train into a siding, change ends and head northbound (on the northbound track). Instead, she changed ends at Canal St and headed north against traffic. Why? Perhaps she didn't understand the instructions. Perhaps she was not as astute as many buffs are about signal placement. Another question was why didn't the C/R. who also has a radio, not pull the emergency cord when he/she realized that the T/O was not following the instructions. From what I found out the crew and 2 managers are sweating this incident out. |

All involved would have been immediately escorted and ordered to report downtown for incident testing and then to submit written accounts of the incident. If there was a "meeting" 2 weeks after the incident it would more than likely been a Step I disciplinary hearing either to demote or terminate the crew. |

The person giving the instructions might have been a bit cryptic thinking there was no way his instructions could be misinterpreted, while the train operator wasn't that familiar with the area and did misinterpret. |

From what I found out the crew and 2 managers are sweating this incident out.Because some don't wanna be branded basically like a snitch, so to speak, as they don't want to commit and action that would "bang their partner in". In my TA days as well as at MNR, I have seen crewpersons who have indeed seen wrong actions, rules violations and the like, and because no one wants to be branded a snitch, or a "company man", ignorance ends up being bliss!! You know this as well as I do. While I never had to do it yet, and can say I have worked with many great people, I am not afraid by no means to pull the plug if things are going wrong and being done wrong. Good thing, again, I have never had to do it yet. |

If that turns out the case, then the C/R will have earned the penalty he/she faces. This was obviously not just a missed station. |

However, that does not excuse the T/O from challenging the instructions. There is no justification for what she did. |
