Another question was why didn't the C/R. who also has a radio, not pull the emergency cord when he/she realized that the T/O was not following the instructions. From what I found out the crew and 2 managers are sweating this incident out.

Because some don't wanna be branded basically like a snitch, so to speak, as they don't want to commit and action that would "bang their partner in". In my TA days as well as at MNR, I have seen crewpersons who have indeed seen wrong actions, rules violations and the like, and because no one wants to be branded a snitch, or a "company man", ignorance ends up being bliss!! You know this as well as I do. While I never had to do it yet, and can say I have worked with many great people, I am not afraid by no means to pull the plug if things are going wrong and being done wrong. Good thing, again, I have never had to do it yet.
