Enforcement is the key.

I wouldn't be at all opposed to other means of enforcement if the police aren't willing to do their jobs -- e.g., a combination of cameras and E-ZPass readers.

Right turns are a problem, since it's not clear exactly where somebody making a right turn is permitted to enter the bus lane, and right turns delay bus service. On Madison Avenue, right turns are banned along the dual bus lane. That's obviously not practical everywhere, but perhaps there could be some sort of split phasing so that cars would never drive in the bus lane at all but would turn across it.

And the typical hours of operation are much too narrow. As long as traffic is heavy, buses should have exclusive use of the curb lane. Turning over the curb lane to, e.g., overnight parking or midday truck loading might make sense in some cases, but in general, the lane should be for buses only.
