There have been several reported instances instances of "runaways" reported. I know that they can happen. Reports of R-42, R-44 and R-46 moving on their own were made during my tenure. All NYCT equipment is designed against a "Single Point Failure" meaning for there to be a train involved in such an event, there has to be more than one failure to occur. Sometimes both are mechanical, sometimes it's a combination of mechanical & human error. It's rarely just human error alone unless there are multiple mistakes made.

There was on incident where an R-46 left the yard and went up the lead before it was stopped - before reaching the main line. 2 RCIs, changing a valve, failed to follow proper procedures. In addition, a required modification to the adjacent "B" car had never been made - several years prior. This allowed the train to charge itself, release the brakes and take power with no one on board. Still, it was human error.
