Being asleep is certainly a possible explanation. By losing track of where he was, I guess I mean "not fully aware". The last station was quite a ways back before the incident -- so plenty of time to become distracted.

I've been in situations where I was making a 60-90 minute drive and either daydreaming or listening to the radio when suddenly I realize I'm a quarter-mile from my exit and not in the correct lane. I wasn't asleep, just not as aware of the situation as I should have been. Of course I have the option of just continuing on to the next exit and doubling back but a railroad engineer doesn't.

Ever since seeing where the cars ended up, I've harbored suspicion that "losing track of where he was" and hitting the curve too fast would be the ultimate cause. Based on the NTSB reports of when the throttle was released relative to when the train stopped, I suspect that is now only a "best case" scenario. Still hard to imagine he would be in a deep sleep and still be able to keep the "dead man" from activating. Also the possibility of a medical situation I guess.
