Presumably some doctor was willing to specify a cause of death (more specific than "natural causes") without doing an autopsy.

Given that, an autopsy only happens if someone insists on it. That could be the medical examiner, the family, or the police. So it's not so much a matter of an autopsy being refused as there not being anyone who asked for it.

A colleague of mine died in a boating accident a few years ago and the family didn't want an autopsy. They were successful in there not being one. I assume they were able to convince the medical examiner that the cause of death was obvious without an autopsy, and therefore an autopsy would not provide any necessary information.
